In enrolling at Ciociaro Club, participants understand that he/she attending the programs and using Ciociaro Club and the facilities does so at his/her own risk. Ciociaro Club and its owners, employees or agents, Sport Fest Windsor volunteers and sponsors shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participant with his/her family in or about any programs on the premises. Participants and parents assume full responsibility for all injuries and damages which occur in or about any programs on the premises, He/She does hereby fully and forever release discharged hold harmless Ciociaro Club, all associated facilities and its owner, employees, agents and Sport Fest Windsor volunteers from any and all claims, demands, damages or rights of action, present or future resulting from any person’s participation in any programs or use of the facility. In addition, he/she agree(s) to follow the rules of conduct and play set by Ciociaro Club and Sport Fest Windsor. Failure to do so may result in suspension from participation. Consent: I the undersigned parent or guardian/participant do hereby grant authority to the staff at Ciociaro Club to render a judgement concerning medical assistance or hospital care in the event of an accident or illness during my absence. I do hereby authorize Ciociaro Club, Sport Fest Windsor volunteers and their assigns to utilize any and all photographs, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials or team films.

Slo-Pitch - $600.00. Deposits are non refundable.
PAYMENT TERMS
$690.00 ($600.00 deposit + $90.00 NSA affiliation fee) due by March 21st. Payment of $350.00 due by April 4th for all teams. Payment of remaining $350.00 due by opening day. Payable by cheque (to Sport Fest Windsor), Visa or MasterCard. Teams with an outstanding balance after 5 weeks of play will be subject to a 2% finance fee per month. Any outstanding account that goes to collection will be subject to a $250.00 administration fee in addition to any legal court costs. All cheques returned NSF will be charged a $50.00 administration fee.
RISK AND WAIVER LIABILTY
The undersigned hereby covenants and agrees for him/her self and any other servant, agent, invites, guest, spectator, organization, employee, member or any other person in any affiliated with him/her (collectively the "Applicant) in the use of the facilities at Ciociaro Club in that they shall be solely responsible for all claims, demands, damages, costs, expenses, actions, causes of action, whether in law or in equity, resulting from any personal injury, or property damage, or for the loss or theft of clothing and equipment, of the Applicant, howsoever caused, and whether contributed to by the negligence of Ciociaro Club, its employees, servants, officers, agents, contractors and/or representative and their successors and assigns. The Applicant covenants and agrees to indemnify and save harmless Ciociaro Club, its employees, servants, officers, agents, contractors and/or representatives and their successors and assigns from and against all liability whatsoever resulting from personal injury, property damage, loss or theft referred to in the preceding paragraph. The Applicant acknowledges that the activities carried on at Ciociaro Club, carry with them an inherent risk of personal injury, property damage and loss or theft of personal property and the Applicant by the execution of the document acknowledges and confirms that he/she is well aware of and understands these risks and is executing and participating in all activities at Ciociaro Club voluntarily. The Applicant further warrants that he/she is in good physical and/or mental health and is able to freely participate in any Ciociaro Club / Sport Fest Windsor leagues, and any health restrictions have been noted. The Applicant covenants and agrees to be bound by all rules and regulations as may be published and provided by Ciociaro Club / Sport Fest Windsor to govern the usage of the facility and all activities conducted thereupon. The Applicant acknowledges and agrees Ciociaro Club and Sport Fest Windsor reserves the sole and exclusive right to use any photographs and/or videos taken during the program for advertising and/or instructional purposes, without cost or charge to Ciociaro Club and the Applicant consents to photographs or videos being taken for the purposes contained herein. The Applicant is responsible for any damages that he/she causes to the Ciociaro Club facility while on the premises. The Applicant further acknowledges that in consideration of being allowed to play in any Ciociaro Club League he/she remains indebted for any amount owing or outstanding by the individuals/teams to Ciociaro Club / Sport Fest Windsor for the league fees. By submitting the above information, I affirm that I have read and understand the rules and regulations of the Ciociaro Club / Sport Fest Windsor Leagues. I also understand that failure to adhere to the wavier noted above could result in disciplinary actions and possibly expulsion from the league, with no compensation for missed games.
